Advance Community Health offers exceptional primary care in Raleigh, NC to every patient, every time. Our Southeast Raleigh location was originally established in May 1997 as a result of the merging of Wake Health Services (our former name) Tarboro Road and Jones Street practices.
In 2015, we expanded access to health care with the establishment of our Southeast Raleigh Medical Campus, a new 35,000 sq. ft, $13 million investment.
As a result of the expansion and merging, Rock Quarry Road Family Medicine, the Horizon Health Center program, New Bern Ridge Pediatrics and the New Bern Ridge Dental Center, all moved into the new Southeast Raleigh Medical Campus and became part of Advance Community Health!
